What address do I put on the application? Please use your best mailing address so we can send any important information, renewal notices etc. to you.  The plan covers you at every address as long as you are abiding by state and local laws.

Can I use one email if I complete several applications for different people? NO, each application (account) must have a unique email address. This is for security reasons as well as to insure errors do not occur.

Can I do my application over the phone? Yes, however we recommend for the most efficient service you complete your application online. Phone, Fax or mailed applications may take longer to process. Online applications are instant with proof sent to your email and in your account center the moment you pay.

 

When going to check out the total is twice as much as it should be. What do I do?

This can happen if you go back and forth on the application. Because of "cookies' many personal computers store the information and then continue to add it when you return to the page. If you find your checkout has doubled in price please close the page, remove cookies (or wait 10min) go back to the site and refresh your browser. This in most cases solves the problem. If you are still having issues you may want to use our print application below.

I completed my application but when I hit submit it returned me back to the application instead of the payment page why? If you are returned to application first check if all the *Mandatory fields are completed. This is the most common error for the application. All fields must be filled or it will return you to the page.

Where can I find a printable application?
